HOLD HARMLESS AND INDEMNITY AGREEMENT (Please Read Carefully Before Signing):

In consideration of the acceptance of this application and the offering of agility training classes to me and my dog, I agree to hold harmless Oregon School for Clever Dogs LLC, Tammy Moody, BAAD, and Cathy Jensen from any and all claims for loss or injury including costs of defense, directly or indirectly to me, my dog, or to my property, while on any part of the school premise, including but not limited to the outdoor arena, driveways, parking areas, and surrounding areas (“Property”). I personally assume full responsibility for the actions and inactions of myself and my dog while on the Property, and I further agree to indemnify Oregon School for Clever Dogs LLC, Tammy Moody, BAAD, and Cathy Jensen from any claims arising from those actions or inactions. I understand I am participating in this activity at my own risk.
